The University of North Carolina Hospitals and Clinics, in conjunction with the University of North Carolina Eshelman School of Pharmacy, offers a one-year specialty residency in cardiology. The residency program is accredited by the American Society of Health-System Pharmacists (ASHP). This postgraduate training program focuses on the development of the knowledge, skills, and attitudes necessary to provide medication therapy management in the area of cardiovascular pharmacy practice. The resident will also have the opportunity to gain clinical, research, and teaching experience in cardiovascular diseases.

The residency provides ten to eleven rotations which can be tailored to the interests, career goals, and experience of the resident. A variety of established practice settings are available throughout the University of North Carolina Hospitals and Clinics.
Required rotations are foundational cardiovascular clinical experiences. These are considered to be areas wherein the resident cannot successfully complete the PGY2 residency without acceptable mastery.
The following clinical rotations are required for successful completion of the residency:
UNC Pharmacy Orientation - 1 month (July)
General Cardiology (Intermediate and Floor areas) – 1 month
Cardiac Intensive Care Unit - 1 month
Advanced Heart Failure – 1 month
Cardiac Surgery/Heart Transplant/LVAD - 1 month
Heart & Vascular Outpatient Clinics - 2 months
Heart & Vascular Outpatient Clinics –½ day per week longitudinal,beginning October 1st of each year. This half day per week will be completed in one of the following clinic areas: heart failure, hyperlipidemia, hypertension, transitions of care, electrophysiology, or general cardiology.
The resident may opt to repeat any of the above rotations during any calendar months that remain after selection of their desired elective months, to a maximum of 3 total months per required rotation. Also, the resident may be required to repeat any of the above rotations (in lieu of an elective month) if it is deemed that their performance during the first rotation month was not acceptable for completion of the residency.
Elective rotations are designed to broaden the resident’s experience in patient populations with unique problems related to cardiovascular diseases or to enhance the resident’s ability to perform in a faculty position after residency.
The following rotations are elective and are optional for successful completion of the residency:

Medicine Intensive Care Unit (MICU) – 4 weeks
Electrophysiology – 2-4 weeks
Nephrology – 2-4 weeks
Academia – 2-4 weeks
The residency year will be scheduled such that required rotations comprise the entirety of the first 6 months of the year. Elective rotations and repeated required rotations will be scheduled in the second 6 months of the residency year.
Individual health insurance is provided. The University of North Carolina Hospitals provides comprehensive malpractice insurance for all residents at no charge; however, this does not preclude the resident from obtaining individual professional liability insurance.
Each resident will be given 15 days of paid time off (PTO) for the entire year, none of which can be taken in the last 2 weeks of the program. These days include all vacation, sick leave, interview days, and professional meeting attendance (beyond the two professional meetings that residents are expected to attend). Residents will work one five-day holiday staffing block around Thanksgiving, Christmas, or New Year’s Eve, and will work one of the three “minor” holidays (the Fourth of July, Labor Day, or Memorial Day).
Applicants must possess a Doctor of Pharmacy degree and must have completed an ASHP-accredited PGY1 residency in pharmacy practice. Applicants without these qualifications will be reviewed on an individual basis to determine the adequacy of their clinical experience. Each resident is required to become a licensed pharmacist in the state of North Carolina.
Pharmacy Practice Responsibilities
Cardiology PGY2 residents will receive training and orientation to the department’s services during the first several weeks of the program. Pharmacy practice responsibilities consist of three 1-week (Monday through Friday) staffing blocks over the course of the residency year and every third weekend (Saturday and Sunday). Responsibilities include, but are not limited to: clinical management of medication issues on the various cardiovascular service lines, verifying medication orders, admission and discharge medication reconciliation, providing patient education, answering complex drug information requests, and participation in all cardiac arrests as the pharmacy member of the team. Residents will also participate in the after-hours clinical pharmacist on-call program. Successful completion of each of the above Pharmacy Practice Responsibilities is required for completion of the residency.
Residency Research Project
The Cardiology PGY2 resident is expected to participate in a project that fulfills the following learning objectives: developing a research question/hypothesis, seeking approval of the research question from the pharmacy and/or medical staff, collecting and organizing data, analyzing results, presentation of the findings at a national pharmacy or cardiology meeting, and submission of a manuscript to a pharmacy or medical journal. Successful completion of a research experience that includes each of the above components on 1 or more projects is required for completion of the residency.
Process Improvement Project(s)
The Cardiology PGY2 resident will be expected to take a leadership role in the improvement of one or more cardiovascular medication-related processes, such as electronic medication order management (order sets, order panels, clinical pathways), medication safety activities, creation of (or updates to existing) cardiovascular medication usage guidelines, and evaluation of new cardiovascular medications for formulary inclusion via Pharmacy & Therapeutics committee approval. Appropriate activities in this category will be identified with the assistance of the RPD and appropriate goals and deadlines (prior to completion of the residency experience) will be set for the resident.
Residency Writing Project (Optional)
Time permitting; the resident may elect to lead the completion of a manuscript comprising a comprehensive review of a relevant cardiovascular pharmacotherapeutic area. This experience will involve idea generation, outline creation, acquisition of collaborators, subsection compilation, manuscript submission, response to reviewer comments, and manuscript re-submission. The resident and RPD will decide during the first quarter of the residency year if the resident intends to participate in this learning experience.
Each resident will earn an appointment as a Clinical Instructor with the University of North Carolina Eshelman School of Pharmacy. Residents are required to participate as a teaching assistant (TA) for at least one Eshelman School of Pharmacy (ESOP) class for either the Fall or Spring Semesters. This class will likely be directed toward second or third year student pharmacists, with a planned emphasis on CV medication education. Successful completion of 1 semester of the above TA responsibilities is required for completion of the residency.
Teaching experiences are also integrated with patient care experiences as part of precepting responsibilities on clinical rotations. Residents will be given the opportunity to serve as intermediate preceptor of 2nd, 3rd, and/or 4th year ESOP students who are completing the same rotation as the resident. Emphasis will be placed on educating the resident on the core precepting principles of teaching, modeling, coaching and facilitating.
Opportunities to lead small-group recitation sessions and participate in didactic lectures in a variety of venues are encouraged. Various teaching opportunities exist within the School of Medicine, Dentistry, Physician Assistant and Nursing curricula; the resident and preceptors will work to identify specific didactic teaching experiences for which the resident may assume responsibility.
Seminars and Professional Meetings
All residents will present a 45-minute ACPE-approved continuing education (CE) seminar once during the year. The ACPE-approved CE presentation is held once on a Wednesday evening and is open for the public to attend. In addition, residents will present 1-2 UNC Case Conferences in their specialty area to fellow residents and students throughout the year. Each resident may choose to attend the ASHP Midyear Clinical Meeting (December) and is encouraged to attend a national cardiology meeting (e.g., American College of Cardiology). Participation in poster sessions is encouraged at these meetings. Successful completion of the above CE and Case Conference requirement is required for completion of the residency.
