PGY2 Pediatric Elective - General Clinical Evenings

Rotation Description

This learning experience is in a multidisciplinary environment whereby the pharmacist provides comprehensive pharmaceutical care through the intensive care units as well as the general pediatric floors. The shift takes over clinical pharmacy responsibilities for all pediatric services including: bone marrow transplant, burn, cardiology, cardiothoracic surgery, dentistry, endocrinology, gastroenterology, general pediatrics, general surgery, genetics, hematology/oncology, immunology, neonatal intensive care, nephrology, neurology, ophthalmology, oral and maxillofacial surgery, orthopedics, otolaryngology, pediatric intensive care, plastic surgery, pulmonology and urology.

Daily activities include:

• Serving as the primary queue verifying pharmacist for all pediatric orders

• Serving as the primary pharmacy representative for Code Blues and Pediatric Rapid Responses

• Serving as the primary pharmacist for pediatric clinical and operational phone calls

• Serving as the primary pharmacist for verifying pediatric home medications

• Participating in evening PICU patient care rounds

• Responding to signouts and monitoring drug therapy to evaluate appropriateness of use

• Maintaining patient profiles within electronic medical record to facilitate communication 

• Practicing antimicrobial management

• Ensuring adherence to formulary and promoting rational, cost-effective drug therapy

• Facilitating discharge and transfer of patients (i.e., medication reconciliation and discharge counseling)

• Reporting adverse drug events and medication errors, when applicable

• Responding to drug information requests and educating staff members, when applicable

• Assisting with distribution activities, when applicable This position is normally staffed/precepted by two pharmacists who rotate weekly, seven days on, then seven days off.
