Rotation Title
Acute Care Family Medicine
Rotation Description:
The UNC Family Medicine Inpatient Service provides acute care services for a wide variety of patients who receive primary care at any of the Department of Family Medicine clinic sites. Learners are expected to follow patients on one of the two rounding medicine teams and interact with the attending physician, medical residents, and medical students on that team to provide comprehensive pharmaceutical care. Rotation responsibilities include, but are not limited to, morning pre-rounding data collection, attendance at morning teaching rounds, participation in daily patient care rounds, admission and discharge medication reconciliation, patient counseling, patient presentations, topic discussions, journal clubs, and provision of in-services to medical, nursing, and pharmacy staff. By the end of this rotation, learners should be able to make evidence-based pharmacotherapeutic recommendations for disease states commonly encountered in an adult medicine population, evaluate medication profiles for patient- and disease-specific appropriateness, obtain medication histories, perform admission and discharge medication reconciliation, provide medication counseling, and use suitable references to address drug information questions.
