The lung transplant/thoracic surgery rotation is an inpatient acute care experience that will focus on utilizing all members of a multidisciplinary team in order to provide optimal care to complex patients. Pharmacists have a role in all steps of the transplant process, from evaluation of patients prior to transplant, to acute inpatient management following transplant, to preparing patients for hospital discharge by providing in-depth medication education for patients and caregivers. Learners will participate in these activities as opportunities arise. This experience is heavily multi-disciplinary and interactions with fellows, dieticians, social workers, and other care teams can be expected in order to best care for patients.
