Pharmacist services are provided 4 to 5 days per week via a protocol-driven Clinical Pharmacist Practitioner agreement. At the conclusion of the rotation, residents are expected to demonstrate the ability to conduct an ambulatory care patient interview, form an assessment, and design, implement, and monitor medication therapy plans targeted to disease-specific outcome goals.
