Rotation Description:

The Carolina Assessment of Medications Program (CAMP) is a pharmacist-led population health clinic that offers comprehensive medication therapy management for a growing number of populations within the UNC Health Care System (UNCHCS). Currently, these populations include high risk members of the UNCHCS Affiliate Health Plan (AHP), high risk members of the UNCHCS Next Generation Accountable Care Organization (ACO), and the uninsured patient population through UNC’s Pharmacy Assistance Program (PAP).  Additionally, CAMP is comprised of the UNC Physicians Network (UNCPN) Anticoagulation Clinic, a pharmacist-led clinic that offers comprehensive anticoagulation management for patients within UNCPN. Patient visits are conducted either face-to-face or via telehealth.
