Rotation Title

Specialty and Primary Care Clinics - Float

Rotation Description:

The UNC Specialty and Primary Care Clinics provide care for patients of all ages and a wide variety of health conditions. All are ambulatory care sites with embedded clinical pharmacist practitioners that focus on management of chronic diseases, assisting with medication access, and monitoring of high-risk medication therapy. Clinics the float pharmacist provides coverage for include: cardiology, endocrinology, family medicine, hepatology, infectious diseases, neurology, pulmonary, rheumatology, and solid organ transplant. Once per week, the float pharmacist provides longitudinal coverage in the transitions of care clinic at UNC Family Medicine Center, which involves medication reconciliation after hospital discharge during multidisciplinary clinic visits. Learners are expected to actively participate in patient care, drug information queries, and administrative responsibilities.
