Population Health/Ambulatory Care Leadership
Rotation Description
The UNC Health Managed Pharmacy Solutions (MPS) rotation is a learning experience in which the resident will gain exposure into population health based principles of care, engage in leadership and stakeholder interactions, and develop skills to engage patients via telehealth. Residents will be exposed to our core subunits: Pharmacy Benefits Management (PBM) services; population health clinical services through the Carolina Assessment of Medications Program (CAMP); Operational Efficiency & Leadership; and Analytics.
The resident will gain exposure to medication coverage and common utilization management programs with the PBM team. The Carolina Assessment of Medications Program (CAMP) is our pharmacist led population health clinic that engages patients utilizing telehealth and face to face visits. Using a combination of predictive analytics, risk scoring, and targeted quality measures, CAMP provides comprehensive medication management and targeted disease state interventions. Our analytics group works closely with all of MPS to provide data that is integral to our infrastructure, core functions, and demonstrated impact. Our operations & leadership groups focus on improving efficiency, aligning core work to system/department strategic aims, and ensuring teammates have knowledge, skills, and resources to be successful in their work.
