Virtual Transplant Orientation Class

Your transplant journey has several steps. Your next one is to watch this video about informed consent for transplant. You need to complete these three steps to move forward in the transplant process. After watching the video, return to My UNC Chart to sign the Transplant Patient Education form. Then contact your transplant nurse coordinator.

Step One

You are required to watch this video, “Welcome to Kidney Transplant Patient Orientation.”

Step Two

When you finish the video, sign in to My UNC Chart and click the Review and Sign button for the Transplant Patient Education form. Look for the green pen icon on the front page or search for the Transplant Patient Education form in the Document Center.

Step Three

Once you’ve signed the form, contact your transplant nurse coordinator. You can find their number at the bottom of your welcome letter, which is in My UNC Chart.
