CSF samples submitted for Treponema pallidum antibody testing are assayed by the CSF VDRL test. The CSF-VDRL is the only test standardized for CSF antibody testing.  Only CSF samples are accepted for VDRL.  Results are reported as nonreactive or reactive.  Reactive results are indicated by flocculation of the cardiolipin-lecithin-cholesterol antigen by antibody.   Reactive samples are titered to endpoint.  While the CSF VDRL is specific for neurosyphilis, it is not sensitive.  Thus, a negative result does not rule out CNS involvement.  Use caution when interpreting results from CSF samples grossly contaminated with blood as they may be falsely positive in the presence of a high serum RPR titer.
