Invitae (LabCorp) Interfaced Orders

• Contact laboratory genetic counseling at LABGC@unchealth.unc.edu) or 984-974-7283 with questions.

• Reflexes and re-requisitions are limited to panels in the same clinical area (orderable via the same interfaced test code) as the initial order.

• Clinic notes or medical records are often required to determine medical necessity of genetic testing.

• Relevant documentation (including but not limited to progress notes and/or genetic counseling notes) must be completed and signed and should be submitted to Invitae within 5 business days to maximize the chance of insurance coverage for genetic testing.

• If the order will be associated with a sponsored testing program, the required form must be completed and submitted to Invitae.

*This test code should be used to order familial variant testing when the proband was tested at Invitae. ALL VARIANTS from the proband report are automatically included. To order targeted testing at Invitae if the proband was tested at another laboratory, order single gene testing using the appropriate interfaced Invitae test code (see above). If there are variants that should NOT be included, make sure this is noted in the order comments.
