Post-Exposure Source Patient Testing Requisition

The appropriate Health Service (see above) must be contacted prior to blood collection. The Microbiology Laboratory must also be contacted (984-974-1805) by the Health Service for ALL exposures to ensure STAT testing. Failure to contact the appropriate Health Service and/or the Microbiology Laboratory may result in delayed testing and delayed treatment.
