Impact of Transition of Care Process for Patients Discharging to A Skilled Nursing Facility (SNF) from a Large Academic Medical Center
My desire to complete my PGY1 residency with UNC Hospitals was multifactorial. Foremost was the Department of Pharmacy’s dedication to implementing innovative practice models. This has allowed for an amazing immersion in delivering advanced pharmaceutical care in both the inpatient and outpatient settings. Second was the sense of community you have throughout the health-system. In all practice settings colleagues are supportive, encouraging and constantly striving to impact the lives of the patients they serve. Next was the direct link to pharmacy leadership who are fully invested in the residency program as well as each unique participant. They strive to ensure well-rounded professional and personal development among not only residents, but also preceptors and program directors. Lastly was the close relationship the program has with the Eshelman School of Pharmacy, which allows for numerous academic pursuits including teaching and research.
