Specimens submitted for RMSF testing are assayed by indirect fluorescent antibody (IFA) testing using a polyvalent conjugate (detects IgG, IgM and IgA antibodies).  IFA is considered the gold standard serologic test for RMSF.  Samples are screened at a 1:80 dilution and tittered to endpoint if positive.  Results are reported as <1:80 or titered to the endpoint.

Antibodies are typically not detected in the first 7 to 9 days after onset of symptoms.  Thus a negative result during acute infection does not rule out RMSF.  The most sensitive approach for laboratory diagnosis is to collect a convalescent sample at least 3 weeks after the acute sample.
